Registered Respiratory Therapist NTE 3 Years
1. Sets up and monitors a variety of complex respiratory equipment such as volume or pressure ventilators for critical care patients. On the basis of patient assessment and analysis of blood gas data, independently makes adjustments to ventilator settings in order to change the patient's ventilation status. Applies advanced ventilator techniques such as pressure control ventilation, tube compensation, volume and pressure support ventilation. Follows established practitioner-driven protocols for weaning patients from ventilators. Monitors patients closely to avoid incidents and to alter the plan as necessary. 2. Initially and continually assesses the cardiopulmonary condition of assigned patients using data acquired though physical examination, respiratory monitoring, and diagnostic equipment. Determines the effectiveness and/or need for change in the therapy being administered. 3. Sets up and monitors oxygen delivery devices such as cannulas, masks, t-adaptors, face tents, humidifiers and aerosols containing prescribed medication to patients using standard respiratory equipment. 4. Performs chest physiotherapy using mechanical equipment or manual techniques. Performs postural drainage to facilitate the removal of secretions . Operates suction catheters attached to a vacuum to remove secretions from patient's airway. 5. Draws arterial blood samples from patient using a needle and syringe, also draws blood from arterial lines and intra-cardiac catheters. 6. During emergency situation s, independently inserts artificial airways into patient's trachea to provide controlled breathing consistent facility policies. During cardiac/respiratory arrests initiates manual ventilations using a manual resuscitation bag and maintains ventilation. During transport of patients, maintain the patient's airway, ensure ventilation, monitor respiration and oxygen saturation, and performs cardio-pulmonary resuscitation when necessary. 7. Performs airway and tube care on patients with endotracheal tubes and tracheostomies including cleaning and securing the tracheostomy and Endotracheal tube and site, changing or re -inserting in emergency situations, monitoring tube cuff pressure and inspecting equipment and patient to insure proper functioning. 8. Records workload, treatments, observations and results of various checks, tests, procedures and education pertinent to patient care. 9. Documents in the medical record, the results of the evaluation including diagnosis and clinical opinion as well as the treatment plan, outcomes of treatment, changes in function al status and patient and family education. 10. Attends and participates in interdisciplinary team meetings where the overall needs of the patient are addressed. 11. Provides inter- and intra -disciplinary in -services to other health care providers. 12. Assists in orientation and training of new staff. 13. Assists in implementing solutions to technical and clinical work problems not covered by precedent or established procedures. 14. Participates in developing guidelines and protocols, new patient programs and evaluation of existing programs. 15. Maintains supplies, equipment and clinical areas to optimize safety and efficiency. 16. May be requested to serve on service-level committees or taskforces or to represent service/section on medical center, network or national committees. 17. Performs other related duties as assigned1. Has a certificate as an RRT from the NBRC (National Board for Respiratory Care) and a registry number, or a certificate from another body which the NBRC recognizes as its credentialing equivalent. This includes registration based on either:
(1) Having fulfilled the requirements and passed the registry examination administered by the NBRC since 1983, or (2) Having fulfilled the requirements and passed the registry examination for a registered inhalation or respiratory therapist administered by:
(a) National Board for Respiratory Therapy from July 1974 through 1982 or (b) American Registry of Inhalation Therapists from 1961 through June 1974. Grade Determinations:
GS-7 None beyond the basic requirements GS-8 Candidates must possess all of the following:
(1) At least 1 year of successful experience related to respiratory therapy at the GS-7 level or equivalent. Qualifying experience at this level is work which includes duties such as:
administering assisted and controlled ventilation to patients with tracheotomies and other complex medical problems requiring frequent adjustments in ventilator parameters; administering and monitoring advanced ventilator techniques such as positive end expiratory pressure and continuous positive airway pressure; developing plans for weaning patients from ventilators; assessing the respiratory status of patients using data acquired through physical observation and clinical analysis of blood gas data, chest x-rays, and electrocardiogram to determine the effectiveness of therapy being administered and to make recommendations to physicians regarding changes in treatment; performing the most difficult respiratory therapy procedures to maintain ventilation, including endotracheal intubation, tracheal lavage, and tracheotomy care; and providing in-service training to others in various specialized areas of respiratory care. (2) Demonstrated knowledge and ability needed to perform complex respiratory procedures with minimal supervision. Is able to participate with physicians and nurses and other staff in planning respiratory treatment, with great reliance on the individual's knowledge of the equipment. (3) Demonstrated knowledge and ability to provide emergency or critical respiratory care; service as a shift leader or supervisor or sole responsible respiratory therapist on a shift; and to plan and conduct training sessions with respiratory therapy students, hospital staff, patients, and family members. PART-TIME REQUIREMENTS. Part-time experience is credited according to the relationship it bears to the full-time workweek (e.g., an RRT who worked 20 hours a week, i.e., half-time, would receive one full-time workweek of credit for each 2 weeks of service).
