Physician Government - Lyons, NJ at Geebo

Physician

The CRC serves the seriously mentally ill (SMI) population of veterans, providing an opportunity to return to the community and facilitating the care and support necessary for these veterans to attain and sustain community-based living using an interdisciplinary care approach. Veterans in the CRC program are not in need of medical or psychiatric hospitalization but are unable to manage their persistent symptoms well enough to live independently. The CRC program is currently comprised of 14 boarding homes who are New Jersey State-licensed, inspected and approved by VANJ, but chosen by the Veterans and/or their families/guardians. All Veterans served must receive medical evaluation for inclusion in the CRC program. These Veterans receive regular medical examinations and primary care visits with the CRC Physician and are seen for outpatient appointments at regularly scheduled intervals as well as on as-needed basis for their complex physical and mental health needs. The Physician's expertise is critical to providing optimal care to the Veterans in the CRC program as their symptoms (medical and psychiatric) change quickly and many Veterans have medical conditions that require constant monitoring and attention (e.g., diabetes, hypertension) for which the homes require regular guidance and oversight. The relationship the Physician develops with Veterans, homes, and CRC team is critical to continuity of care, stability and to successful community living and is built on regular (at times weekly) contact and same day access with the same provider over time. This level of outpatient medical care for SMI Veterans cannot be achieved in typical primary care settings and is essential to avoiding hospital recidivism and adverse outcomes. As noted above, the Physician also serves as Collaborating Physician for the 2 Medical Nurse Practitioners and 4 Physician Assistants, all who work in either MH&BS inpatient unit, residential care units (over 100 beds serving a complex array of Veterans with diverse and, at times, complex medical needs), or the Bound Brook Transitional Residence in Homeless Services. The Physician attends the monthly Ambulatory Care meetings and, as necessary, provides direct medical coverage for the inpatient Mental Health unit. Work Schedule:
